Information needed for a waiver
CHAIRS
In order for a waiver for a chair to be considered you must first have an evaluation from Frank Gonzales, Environmental Health Services, phone# 491-2724. Frank will then assist with the documentation for the waiver.
OTHER
When requesting a waiver you must first contact our CCI representative to see if they can meet your needs and then you will need to write a memo to Farrah Bustamante in the Purchasing office. The memo must include the following:
- Quantity needed
- Complete description & specifications, not simply a brand and model number
- Quoted unit price
- Quoted total price F.O.B. DESTINATION
- Picture or sketch of product (if available)
- Specific reasoning for waiver
The Purchasing Office will take your information and complete a waiver form for the Director of Purchasing to reveiw. An approved waiver will then be submitted to JVP (CCI). JVP (CCI) will then send their reply to the waiver and you will be notified.
